SEBA’s LASIK Eye Surgery Reimbursement Program

If you are a SEBA Active member*, you are eligible to receive up to a $1,000 one-time reimbursement after your LASIK surgery!  After your LASIK surgery is completed by the doctor of your choice, send us a copy of your receipt indicating you are the patient and what services were rendered and, upon verification, we will mail your reimbursement to you within 2-3 weeks.
